New Patient Registration

The doctors welcome new patients who live within our practice area. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a registration form, available at reception. Please note a seperate form for each family member is required.

Should you choose to register using the the Paper version of the PRF1 form, please drop into our friendly reception team, email into us on info.greenridge@nhs.net or post to us at:

We aim to have you registered within 5 working days but please allow up to 2 weeks for your registration to be completed. You will receive an SMS to confirm you are successfully registered with us a Poplar Primary Care Centre.

Once registered, if you feel you need medical advice, need to see a GP, have a query regarding your medication or need to order a Fit to Work Note (sick note) please submit an AccuRx Triage, simply complete the form. We will hope to respond to your request either by text message or telephone very quickly. Appointments are offered on a Face to Face basis once your triage request has been reviewed by our Triage Doctor.
